Clinical Observation of the Stumps of 83 Leprous Below-the Knee Amputees

Abstract
The stump of amputated leg in leprosy is insensitive due to sensory disturbances and is prone to develop many complications upon prosthetic fitting. A clinical observation has been done on stump complications in 83 leprous amputees who received below-the-knee amputations.
1. The amputated stump of leprosy patients showed severe degree of muscle atrophy, atrophy and resorption of the local bones in addition ulcers of covering skin, were quite annoying and hindered proper gait as well as prosthetic fitting.
2. Skin ulcers around the stump were 102 in number and their location was found to be at the bony prominence of the stump such as the fibula head, the medial condyle of tibia, and the tibial tuberosity. It is apparent from this study that the locations is not related to the contact area within the socket of prosthesis.
3. The Decancellation of head with shaving of the shaft of the fibula was. done in 19 cases and in all it was found to be effective for healing and preventing the ulcer in this region, the most favorite site of this lesion, with this result it is justifiably proposed that the amputation itself should be supplemented with this procedure.
4. It has also been noted that for leprous amputee the improvement of living standard, and strict adherence to the stump hygiene should be exercised and that a more realistic prosthetic device should be innovated.
